    Facsimile signature ‘J Cooke MD FRS’ on separate paper and inscribed ‘J.COOKE MD FRS. 1821’ on paper support.

    John Cooke (1756-1838) was a British physician, he was elected a Fellow of the Royal Society in 1821.
